Rejected Twice Due to Image Quality

 

Q:  When can I request a Georgia name check?

A:  A name check can be requested when the fingerprints have been rejected twice for image quality by the Georgia Bureau of Investigation’s (GB) Automated Fingerprint Information System (AFIS).

 

Q:  Who can request a Georgia name check?

A:  Any agency or business that already has legal authority to submit fingerprints to the GBI for non-criminal justice purposes.

Q:  What do I need to include with my Georgia name check request?

A:  You will need to provide the following information when completing the form –

 

·         The Transaction Control Number (TCN) from both rejected transactions, Name, Date of Birth and Social Security Number (if available). 

·         The Originating Agency Identifier (ORI) or Originating Agency Code (OAC) of the agency that the results are being returned to.

·         Contact information for the agency including the agency’s preference for receipt of the results – FAX or Mail.

 

Q:  What do I need to include with the Name Check Request form if I am authorized to receive a federal check in addition to the Georgia check?

A:  If your agency has an ORI Number and is authorized to receive a federal criminal background check on the applicant, you must submit to the GBI two(2) hard copy FBI cards with your agency’s ORI Number and the applicant’s fingerprints on the cards with the completed Georgia Name Request form.

Q:  How do I get fingerprints on the hard copy FBI applicant cards?
A:  The agency should provide the applicant with the following

·         Three (3) FBI Applicant Fingerprint Cards

·         Cover Letter to Local Law Enforcement

·         Self-Addressed Stamped Envelope with Agency’s Address

 

The cover letter to local law enforcement should explain that the applicant’s fingerprints have been rejected due to poor quality through the Georgia Applicant Processing Services (GAPS) and the agency is requesting the law enforcement agency to roll the applicant’s fingerprints on the 3 FBI applicant fingerprint cards.  In addition the agency should request that the 3 FBI cards be sent to the requesting agency in the self-address stamped envelope that has been provided.

Q:  What happens if the FBI rejects the fingerprints that were submitted by GBI?

A:  If the FBI rejects the hard copy fingerprints that were submitted by the GBI for image quality, the agency is responsible for sending the FBI a hard copy card with the applicant’s fingerprints.  (Note:  This card should be sent directly to the FBI; do not send to the GBI.)

 

If the agency receives a second FBI reject for the transaction for image quality, the agency should follow the instructions for completing the FBI Name Check Request.

 

Q:  What information will I receive from the Georgia name search check?

A:  If candidates are generated during the name search check and the candidates are identified to an existing criminal history record, the agency will be provided a copy of the criminal history record.  If no record is found, the fingerprint card will be stamped “No Record” and returned to the agency.  

 

Q:  Is there a time limit for obtaining these Georgia name checks?

A:  Yes.  The name check request must be submitted within 90 days of the last (second) rejection date.
